Due to the COVID-19 pandemic, the State of Illinois now allows for medical cannabis consultations to be conducted via telemedicine.

I have seen first-hand the positive impact medical cannabis has on the quality of life of my patients. This is why I have been helping patients access medical cannabis since the program began in 2015.

The Process

Get an Illinois Medical Cannabis Card application completed in just one visit!

We'll send you a link to set up an appointment for your initial consultation.  We can usually see patients within a week's time.

Almost every patient receives their signed physician certification after their very first consultation.

We charge $300 for patients who receive a medical certification.

IMPORTANT If you do not qualify you will not be charged anything.

Our support staff will help you complete your application at no additional charge.

However, you will have to pay the Illinois Department of Public Health $100 for a 1-year card, $200 for a 2-year card, or $250 for a 3-year card.

What other requirements are there to get a card?
A.

You must be able to demonstrate that you are an Illinois resident. You can demonstrate residency with either a driver's license or with official mail in your name sent to your Illinois address.

Do you accept insurance?
A.

No. All expenses are out-of-pocket.

How old do I have to be?
A.

There is no age minimum. Minors (under 18) will need parental consent and will also require a second doctor's certification, which will require an additional doctor's visit and cost an additional $100.

What if I'm not sure that I qualify?
A.

Fill out the form with the information you have and let's figure it out together. Many more people qualify than believe they do and, with a free initial consultation, there's no risk to you in trying.

How long will it take to get my card?
A.

You will get your provisional medical cannabis card the day after your appointment with me. Anyone with an opioid prescription can receive their card the same day. You will be able to purchase medical cannabis using your provisional medical cannabis card while you wait for your State-issued card to arrive in the mail.
