Almost every patient receives their signed physician certification after their very first consultation.
We charge $300 for patients who receive a medical certification. For established patients seeking renewals, we charge $250.

Our support staff will help you complete your application at no additional charge.
However, you will have to pay the Illinois Department of Public Health $50 for a 1-year card, $100 for a 2-year card, or $125 for a 3-year card.
You must be able to demonstrate that you are an Illinois resident. You can demonstrate residency with either a driver's license or with official mail in your name sent to your Illinois address.

There is no age minimum. Minors (under 18) will need parental consent and will also require a second doctor's certification, which will require an additional doctor's visit and cost an additional $100.

You will get your provisional medical cannabis card the day after your appointment with me. Anyone with an opioid prescription can receive their card the same day. You will be able to purchase medical cannabis using your provisional medical cannabis card while you wait for your State-issued card to arrive in the mail.
